Paperwork checklist

Use this checklist to make sure you have all of the required paperwork for the show. You are required to submit copies of all necessary paperwork with your entry, if you don't you will be charged a $5 processing fee per entry needing copies. You should bring your originals to the show. 

So don't leave home without your paperwork! :o)

 

All Puppy classes:

• Proof of canine's current vaccinations: either a health certificate signed by a licensed vet; or a copy of the original vaccination record. 

Youth, Young, and Adult:

• Proof of canine's current vaccinations: either a health certificate signed by a licensed vet. or a copy of the original vaccination record.
• Proof of dog's USA registration: either original pedigree, notarized copy of original pedigree, or letter of pending registration from USA.
• Proof of current membership in USA: either a copy of USA membership card or mailing label off of Schutzhund USA magazine. If you are a resident of the United States of America and own a dog (12 mos. or older) entered in the show.

Working Classes:

• Proof of canine's current vaccinations: either a health certificate signed by a licensed vet. or a copy of or the original vaccination record.
• Proof of dog's USA registration: either original pedigree, notarized copy of original pedigree, or letter of pending registration from USA.
• Proof of Breed Survey Report: either original or notarized copy of original. If dog has already been breed surveyed or is over 3 1/2 yrs. old vying for the rating of excellent.
• Proof of hip certification: either the "a" stamp on the pedigree or proof of OFA certification. If dog entered is vying for the rating of excellent.
• USA recognized scorebook with record of the dog earning either at least a SchHI, IPI, or an HGH.
• Proof of current membership in USA: either a copy of USA membership card or mailing label off of Schutzhund USA magazine. If you are a resident of the United States of America and own a dog (12 mos. or older) entered in the show.

Breed Survey

• Proof of canine's current vaccinations: either a health certificate signed by a licensed vet. or a copy of or the original vaccination record.
• Proof of dog's USA registration: either original pedigree, notarized copy of original pedigree, or letter of pending registration from USA.
• Proof of Breed Survey Report: either original or notarized copy of original, if the dog is entered for any type of survey other than a first-time survey.
• Proof of hip certification: either the "a" stamp on the pedigree or proof of OFA certification.
• USA recognized scorebook with record of the dog earning either at least a SchH 1, IP 1, or an HGH.
• Proof of passing of an AD recorded in their scorebook.
• Proof of at least a G show rating: at least one show card from a USA recognized conformation event. The minimum rating accepted is that of G (Good).
• Proof of current membership in USA: either a copy of USA membership card or mailing label off of Schutzhund USA magazine; If you are a resident of the United States of America and own a dog (12 mos. or older) entered in the show.
